I use Abraham Verghese's My Own Country: A doctor's story, in my Biology and Literature of HIV/AIDS class.  It is a physician's look at 
HIV/AIDS in Apalachian Tennesse.

I haven't used these, they were advertized on a health professions list serve:

major crafter of the OR Health Plan
