RE: Volunteer (prerequisite vs DWF data) [HAPP-L]

As I understand it, our goal is to find out

a) what percent of 2 and 4 yr colleges stipulate bio and/or chem as a
prerequisite for A & P

b) whether schools that have some sort of prerequisite have a lower rate
of students withdrawing or failing A & P.

So the question is really about prerequisites, and this will have to
suffice as an indirect way of getting at the question of whether taking
biology and/or doing well in biology might affect performance in A&P. It
would be much more
complicated to include information on specific students, the courses they
took, and how they did.

The form looks easy enough for us to fill out. But I have one question.
Some of my students have taken biology or chemistry previously and some
have not. How should that be handled?

I've been making online forms for my course evaluations, and I can put one
up for the HAPS survey. The results get automatically formatted into an
Excel spreadsheet, which saves the time of entering them by hand. I can
then forward it to anyone who wants to analyze it.

I just made something tentative - not "live", so you can't submit it, but
when everyone decides what it should look like, I can edit it and then add
the Submit button:
