Ric, I just submitted the survey form. After I clicked on the submit button, it didn't show any window that confirmed the submission - the only thing that happened is that it brought me back to the "Login" window. I hope that my submission went through.

I hope everyone has had a wonderful holiday season and that you are 
ready for a joyous New Year.  This note is to ask that if you have 
not yet filled out the accreditation/course survey form on the 
website for your school, it would help HAPS a lot if you would take 
the time to do so as soon as possible.  I think it is very important 
for HAPS to be able to describe the profile of instructors teaching 
the course now, as part of our ongoing discussions with SACS. To date 
we have received data for 45 schools, collectively teaching just over 
21,000 students per year.  That probably represents something on the 
order of 5-7% of the US student population in this course, but in 
terms of instructors that gives us an 'n' of only 131 for full timers 
and 78 for part-timers - roughly one faculty for every 100 students 
(no wonder everyone is busy....).  I'd really like to increase that 
number before drawing any conclusions. To go to the online survey,

If your number of faculty exceeds the number of allocated spaces 
(even on the expanded form) just fill out the survey twice.  Since I 
am manually extracting the information, I will just combine the 
numbers.
The data are being grouped by region, but at this point the n is too 
small to do any comparisons among the regions, and we are looking 
only at total faculty and numbers of degrees held in each of the 
major categories.
