WRT cost, I always like to tell the story of when I
was doing competitive bicycling: If I saw a new rider without a
helmet, I'd drop back and talk with him (it was usually a "him.") Then
I'd get around to suggesting a helmet. There was a predictable series of
responses like fit, comfort, heat transfer, et myriad cetera, but it often
came down to cost. At that point, one of the other riders would
often drop back, enter the conversation, and offer his business
card.
He was a neurosurgeon, and happily pointed out that his
fees in the OR ran between $90 and $120 per
minute.
