You will receive 5 points extra credit for successfully completing this form.   This survey is an additional extra credit for you and you still have your 2 extra credit requests as stated on the course syllabus.  Please make sure that both your Name and Email address are entered properly.

Explanation of Survey Questions
3.  Personal Computer Do you have access to a computer at your home?
4.  Computer Platform This refers to the operating system on your home computer. You may check both if you have both a PC and Mac at home.
5.  Software Check any software programs on your home computer.
6.  Primary Internet Access For your class work, do you have access to the Internet from your home, at the house of a friend, through a Palomar computer lab, or no access at all?
7.  Internet Access Speed What is the speed of the Internet connection that you use for your class work?
8.  Your Computer Skills How do you rate your computing skills?
Beginner - Using a computer 6 months or less, for word processing and web browsing.
Moderate user - Using a computer more than 6 months for word processing, web browsing, class research projects.
Power user - Using a computer for word processing, and/or spreadsheets, PowerPoint, web design, graphic design.
